EIC Extra Trophy Form

This form is to purchase additional EIC Trophies for other partners involved in the winning project. Please fill out this form and submit. We order trophies on the first of each month. Your additional trophy will be shipped to our Charlotte office and you will be notified when they have arrived. Delivery time can be up to 4 months due to international shipping. All costs include shipping and handling. Payment must be received prior to ABCC placing the order. Payment can be by c/c purchased through this form or check sent to our office. ACH information is available if you prefer that route of payment - please contact freeman@abccarolinas.org for that form.
